Object to the decision (odwołanie) — Polish letter template

Use this if you think the decision is wrong. Send it signed, before the deadline, to the address in the instructions (pouczenie) — for a decision, usually through the office that made it. Give your reasons and keep proof that you sent it, for example the receipt for a registered letter.
